As of late 2017, my private practice is closed to both new patients and any further clinical work with previous patients. If you have been a recent private patient of mine, you can still sign in using the user ID and password you were assigned. If you have forgotten, or have not received a user ID, please call the office at 413-230-3541 [or the old "backup" line 413-570-0425] to obtain this.

My clinical work now primarily involves deep transcranial magnetic stimulation (dTMS) for patients with "treatment-resistant" depression. I carry out this work in the offices of Achieve TMS East, headquartered in Northampton, MA (877) 297-5816.

Previous Instructions:

** For easy reference, please "Right-Click" on this link now - http://drzebrun.net/portal-info - and select "Open in New Tab" or "Open in New Window" to have a copy of these directions on hand when you log in for the first time.

Please do the following:

1) Login by selecting the "Patient" option (or the "Collateral..." option if appropriate) that you see to your right. You will then be taken to the HOME screen where you will see three options on your left.

2) If this is your first visit, please click on "CHANGE MY PASSWORD" and change your password immediately to something that you can remember but would be very hard for others to guess. You will be prompted to select some password reminder questions.

3) Then, back on the home page, click on "MESSAGES" and click on any links that you find on the messages page.

[Note that there are "back" and "home" buttons at the top of pages which you can use to navigate]

4) Finally, from the home page, click on "MY TO DO LIST" and read and complete each form I have initially placed there for you. After reading or filling out each required form, scroll down to the bottom of the form, enter your password if requested, and click "SIGN AND COMPLETE DOCUMENT."

The information you submit is secure and no one else has access to your clinical information as it is password protected.

For clients under the age of 18, a parent or guardian will need to fill out each form for the client and electronically sign each form.
